Use this editable template to record behaviours of interest and concern before, during and after an incident to understand why a particular behaviour is happening.
This template also includes space to record possible functions for the behaviour, changes to individual care required, and updates to individual support plans.
Please note: Functional analysis can be useful in understanding why we are seeing a particular behaviour, allowing us to put in place effective supports. However, it does not consider experiences that may be driving the behaviour.
